Job Description Landmark Hospital of Savannah has an exciting opportunity for a PRN Physical Therapist (PT) to join our dedicated and professional team! Shift: PRN Summary: A Physical Therapist (PT) is responsible for the assessment, planning, implementation, treatment and evaluation of physical therapy for patients. Responsibilities Knowledge of specific conditions, diseases, and treatment therapies for specific patient populations Assesses patients' physical, psychological, social, and educational needs to determine level and type of care Develops and maintains current patient treatment plan to include patient problems, goals, and interventions Delivers care based on acuity level, identified needs, and physician orders Coordinates physical therapy care activities with other departments/disciplines Assesses, collaborates, and coordinates patient's discharge planning needs and readiness for discharge Assesses patient/family learning needs, abilities and readiness to learn and integrates needs into plan of care Provides appropriate education/orientation to patients/families Requirements Bachelor's degree or post Graduate degree (PT) Active Physical Therapist (PT) License in the state of Georgia BLS and ACLS Certification Minimum 1 year hospital experience, critical care/acute care preferred Excellent communication and interpersonal skills Superior analytical, problem solving and critical thinking skills Team, detail and goal oriented Positive, professional and empathetic demeanor About us We are looking forward to talking to you about the opportunities available at Landmark Hospitals.
