Methodist Le Bonheur Healthcare is seeking an Alliance Liaison to enhance continuity of patient care by coordinating collaboration among hospital teams, hospice/home care services, and post-acute facilities. You will develop and maintain partnerships to improve Alliance referrals and support care transitions for patients and families.
