Registered Nurse – Home Health

Shift: 8 hours

Contract Length: 13 weeks Guaranteed Hours: 40

Requirements:

Current state license as a Registered Nurse
1-2 years experience as a Registered Nurse in a clinical care setting
2+ years home health experience
Homecare Homebase Software Experience
OASIS experience
Proof of current CPR certification
Current driver's license
Reliable transportation and insurance

Roles & Responsibilities:

Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
Educate patients and their families regarding care, medications, diet, and home safety, promoting self-care practices.
Supervise and provide clinical direction to support staff to ensure quality of service.
Monitor assigned cases to ensure compliance with third-party payer requirements.
Provide on-call services to patients and their families as assigned.
