Hospice Chaplain

Transitions Care seeks a PRN Hospice Chaplain to provide spiritual support for patients and families in Ottawa, Streator, Dwight, Kankakee and surrounding areas. You’ll serve as the spiritual anchor within the interdisciplinary team, guiding care with compassion and cultural sensitivity.
