Advance your nursing career with a rewarding field-based role supporting individuals in their homes. This full-time, 13-week contract offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, providing consistency while making a positive impact on patient outcomes. Youll conduct 3 to 5 patient visits daily (targeting roughly 30 points per week) and be reimbursed for mileage traveled within your assigned territory.

Key Responsibilities:

• Complete comprehensive OASIS assessments and formulate individualized care plans.
• Deliver skilled nursing services: wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate with the care team and physicians.
• Educate patients and families about illnesses, treatment plans, medications, and preventive care strategies.
• Document care efficiently and accurately using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated patient care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Maintain compliance with regulatory and agency standards for care delivery.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least one year of recent Home Health experience (field experience highly preferred).
• Proficiency in Home Care Home Base (HCHB) is required.
• Strong familiarity with OASIS documentation tools.
• Must possess reliable transportation, a valid drivers license, and current auto insurance.
