This position is located onsite at our state-of-the-art Rehabilitation Center in Riverhead, New York. Key Responsibilities

• Conduct thorough evaluations of residents to ascertain their physical capabilities and specific treatment needs, utilizing evidence-based assessments.
• Design and implement personalized treatment plans tailored to enhance each resident's strength, mobility, and overall functional independence.
• Administer therapeutic exercises, manual therapy techniques, and other modalities to effectively address physical impairments, ensuring the highest standards of care.
• Regularly monitor and document residents' progress, making necessary adjustments to treatment plans to meet evolving needs and enhance recovery.
• Engage and collaborate with an interdisciplinary team to provide holistic, multifaceted care that addresses all aspects of the residents' health.
• Educate residents and their families on at-home exercise strategies and lifestyle modifications to support ongoing recovery and physical health.
• Uphold compliance with all state regulations and federal standards, maintaining a commitment to the ethical practice of physical therapy.

Patient Population / Client Focus

The successful applicant will primarily work with geriatric residents, many of whom may be recovering from surgeries, managing chronic conditions, or seeking rehabilitation after hospitalization. This requires a deep understanding of the unique needs of older adults, excellent assessment skills, and the ability to forge lasting rapport.

Qualifications

• Valid Physical Therapist licensure in the state of New York.
• Bachelor's Degree in Physical Therapy or a related field, with a strong preference for candidates holding a Master's Degree or Doctorate from an accredited institution.
• Proven competency in physical therapy evaluation and treatment techniques.
• Strong organizational, communication, and interpersonal skills vital for effective collaboration and patient interaction.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, particularly within a skilled nursing facility or geriatric care setting, is highly valued.

Compensation and Benefits
