Hem/Onc only practice
Adults only
No Radiation Oncology on-site, but connection to Rad Onc team at Loyola University Medical Campus, which is 5 miles from practice.
No APP's in practice
Documentation System/EMR: epic
Estimated Credentialing Timeframe: 3 months

Practice Setting: Outpatient Free-Standing Community Cancer Center

Average Number of Patients Per Day: 16-20

Trauma Level/Designation (Hospital): Level 2
