Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional and social needs of clients and their families, facilitating personalized care plans.

• Engage in ongoing education to remain informed about best practices in social work, particularly concerning hospice care and gerontology.

• Facilitate family meetings when necessary, mediating discussions to resolve conflicts and support family dynamics around patient care.

• Educate clients and their families on available support services, enhancing their community engagement and social connections.

The primary focus of this position is on a geriatric population within a hospice care framework. Qualifications\n
