Overview
Physical Therapist – Home Health in Belle Fourche, South Dakota. This travel assignment starts 07/20/2026 and runs for 13 weeks. Weekly pay range is $2,292 - $2,467 per week, with a 40-hour weekly guarantee. Shift is Days, in a long-term care setting (LTC – Nursing Home).

Responsibilities
- Assess residents’ physical therapy needs using standard evaluation tools; develop and implement individualized plans of care.
- Deliver skilled PT interventions per physician orders, including therapeutic exercise, gait training, balance, and functional mobility activities.
- Re-evaluate progress regularly; adjust treatment plans and goals based on outcomes and interdisciplinary input.
- Document all encounters, treatment plans, progress notes, and outcomes in the resident’s medical record with accuracy and timeliness.
- Educate residents and families on home exercise programs, safety strategies, and discharge planning.
- Collaborate with the interdisciplinary team to optimize mobility, safety, and quality of life; participate in care conferences as needed.

Requirements
- Active Physical Therapist license in South Dakota or eligibility for SD licensure.
- Graduation from an accredited physical therapy program; Doctor of Physical Therapy (DPT) degree preferred.
- Experience in geriatric, skilled nursing, or home health settings is preferred.
- Ability to work in a fast-paced long-term care environment; excellent communication and documentation skills.
- Willingness to participate in a travel assignment and adapt to new facility workflows.
