Hospice Registered Nurse

Location: Chittenden & Grand Isle Counties, Vermont

Assignment Duration: This is a 13-week contract position.

Schedule: 10-hour day shifts; Weekend shifts required as needed.

Job Description: We are seeking a dedicated and compassionate Hospice Registered Nurse to provide end-of-life care, specializing in symptom management and education. The role involves working primarily in home health settings as well as in a respite house, interacting with terminally ill patients and their families.

Responsibilities:

• Conduct comprehensive assessments and create care plans.
• Manage patient's complex symptoms with high-level critical thinking skills.
• Facilitate goals of care discussions with patients, families, and team members.
• Administer medications via various routes including IV, subcutaneous, oral, and rectal.
• Perform procedures such as central line care, wound care, and pain management.
• Educate patients and caregivers on end-of-life care and symptom management.

Required Experience / Certifications / Licensure:

• A minimum of 3 years of experience in community hospice care.
• Current BLS certification required.
• Experience with electronic medical records (EMR), specifically EPIC within the last 2 years.
• Strong typing skills for detailed documentation.
