Responsibilities
- Provide direct nursing care to residents according to the plan of care and physician orders.
- Administer prescribed medications and maintain accurate documentation of administration, responses, and any adverse events.
- Monitor resident conditions, perform routine assessments, and report changes promptly to the supervising nurse and care team.
- Assist residents with activities of daily living and support comfort, dignity, and safety during care.
- Collaborate with the interdisciplinary team to update care plans, participate in care conferences, and ensure continuity of care.
- Uphold infection control standards, documentation accuracy, and compliance with health and safety regulations.

Requirements
- Active LPN license in Pennsylvania (PA).
- CPR/BLS certification.
- Ability to work 40 hours per week; available for day and/or evening shifts.
- Experience in or exposure to long-term care or geriatric nursing is preferred.
- Willingness to participate in a travel assignment and relocate as needed.
