Duties: • Complete admissions to home health services and perform comprehensive patient assessments. • Provide case management and coordinate individualized plans of care. • Provide oversight and guidance to LPNs as part of the patient care team. • Evaluate wounds and support appropriate wound care management. • Maintain timely documentation and communicate effectively with patients and the healthcare team.
