Agency Collaboration & Referrals: Consult with internal management, statutory/voluntary agencies, and caregivers to ensure treatment compliance while connecting participants to community health, welfare, and social resources. Complex Case Management: Support concurrent reviews, discharge planning, home care, and ongoing social service coordination by building strong networks and identifying creative solutions for participant needs. What Minimum Requirements You’ll Need Licensure / Certification / Registration: Preferred Credential(s): Drivers License. Masters Level Social Worker. Education: Master's degree in Social Work required. Work Experience: 3 years of experience preferred.
