Responsibilities Identifies patients in need of social work intervention within 24 to 48 hours of admission based on high risk screening criteria. Follows up on all referrals made to social work from all sources other than high risk screening within 24 hours of referral. Assesses patients to determine the bio-psychosocial factors that impact their medical condition, treatment, recovery, and safe transition from one care environment to another Develops intervention plan and discharge plan to meet patient’s/family’s needs. Provides social work services to patients in OB and takes weekend on call for OB/emergency department. Other duties as assigned. Qualifications Required: Education: Master’s degree in Social Work Experience: One year experience in an acute inpatient hospital setting or equivalent Experience Licensure/Certification: LCSW eligible.
