Senior Social Worker - Care Management & Resource Coordination

Blue Shield of California is seeking a Care Management Clinical Social Worker to assess members with complex social determinants of health, develop individualized care plans, and connect them to appropriate services. You will collaborate with Care Management teams, BSC nurses, and providers to coordinate care and improve member outcomes.
