Hospice Triage RN - Remote Field Staff - Supportive Care (Fully Remote)
Moments Hospice
- Serve as a key member of the interdisciplinary team providing after-hours clinical support to patients and families.
- Work fully remote using Telecare protocols to assess needs and coordinate care.
Key Responsibilities:
- Perform triage, clinical assessment, and interventions via phone.
- Support patients and families with symptom management and end-of-life care needs.
- Collaborate with physicians, facilities, and internal teams to ensure positive outcomes.
Qualifications:
- Active RN license with no current or pending restrictions.
- Hospice experience and familiarity with Home Care Home Base (HCHB) software.
- Strong clinical judgment, critical thinking, and communication skills.
