Nurse Practitioner: Chronic Care & Longitudinal Primary Care
Healthonomic Primary Care
Healthonomic Primary Care seeks clinicians with active Illinois APRN and RN credentials and board-certified FNP-BC to lead chronic care management and longitudinal primary care. Experience in RPM/CCM programs and value-based care concepts is advantageous.
