Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. MINIMUM REQUIRED QUALIFICATIONS – TN/compact RN license BLS Stroke Certification 2 years case management experience in an acute hospital setting Acute hospital experience during the past year with no more than a 90-day break Level I trauma center experience PREFERRED QUALIFICATIONS – ACMA LENGTH OF ASSIGNMENT – 12 weeks SHIFT / HOURS PER WEEK – 8:00AM - 4:30PM SYSTEMS – Meditech, Midas, Careport START DATE – 11/2/2026 Facility Location Filled with Southern charm, scenic beauty, boot-scootin’ dance halls and country music at every turn, “Music City, USA” is always a fascinating destination.
