Licensed Vocational Nurse (LVN)

Our home health and hospice agencies provide personalized in-home care to communities throughout California, Hawaii, Oregon, and Washington. We are committed to delivering specialized nursing care and related services under the supervision of a Registered Nurse (RN) or Medical Doctor (MD).

Job Summary

The Licensed Vocational Nurse (LVN) performs nursing procedures that require specialized training for assigned patients in accordance with established policies. This role includes participating in the planning and provision of patient care while utilizing knowledge of asepsis in performing treatments.

Essential Functions

• Provide care for assigned patients under the supervision of an RN according to established standards and nursing processes.
• Conduct routine visits in compliance with departmental policies and procedures.
• Assist in developing a prioritized interdisciplinary plan of care for each patient.
• Implement and coordinate the plan of care utilizing appropriate organizational skills.
• Utilize individual teaching plans, including Patient Teaching Protocols and discharge Plans, focused on patient decision-making.
• Perform assigned clinical procedures, such as phlebotomy, injections, and drug screenings.
• Administer medications as prescribed.
• Order and restock supplies as needed, adhering to established par levels.
• Maintain cleanliness and organization in assigned areas.
• Perform other job-related duties as assigned.

Job Requirements

Education and Experience

• High School Education/GED or equivalent: Preferred
• Bachelor's Degree or equivalent combination of education and related experience: Preferred
• Minimum two years of recent experience in the field: Required

Licenses and Certifications

• Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
• Basic Life Support (BLS) Health Care Provider certification from an approved vendor: Required
• Arrhythmia certification: Preferred
• Valid Driver's License (DL) and must be at least 21 years of age: Preferred

Organizational Requirements

The organization prioritizes the safety and wellbeing of associates and patients, which includes requiring all associates to receive all mandatory vaccinations as a condition of employment and annually thereafter, where applicable. Medical and religious exemptions may apply.

Additional Notes

This position may require flexibility, including weekend shifts and working three days during the week as operational needs dictate. Experience with electronic medical record systems such as EPIC and recent hospice care experience are highly desirable.
