Care Coordinator Float Part-Time

Erie Family Health Centers, a nationally recognized top workplace with 14 sites in Chicago and suburbs, is looking for a valuable addition to our Patient Program Department! The Social Drivers of Health (SDOH) Care Coordinators at Erie connect patients with a range of services to address the social drivers of health. The Care Coordinator role is a direct service patient-supporting role, with some administrative responsibilities. The Care Coordinator supports patients in-person in the clinic and works directly with clinical care team members. The Float Care Coordinator ensures consistency of SDOH services across their assigned region by providing on-site support at any of the health center sites in their region as required. Responsibilities Attend to patients through warm handoffs from the clinic. Screen clients for needs around social drivers of health and connect to internal and external resources. Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc. Administrative Responsibilities : Complete data entry and documentation. Assist with distribution and tracking of SDOH resources, including bulk mailing of SDOH resources. This also includes Top Box Foods distribution, ordering Uber Health rides, and distributing other material resources. Use Epic to document work, respond to requests, process work queues, and communicate with clinical care teams. Performs other duties as assigned. Qualifications Education Required: High School diploma or equivalent required. Preferred: Associates degree or Bachelor’s degree a plus Experience Work experience in community health, social or community services, or comparable experience preferred. Skills And Knowledge Required: Verbal and written communication skills, including high level of comfort presenting to groups. Comfort using telephonic interpretation services required. Ability to operate various office equipment and proficiently use common software applications for email, word processing, and data management. Preferred: Intermediate proficiency in Microsoft Office preferred Bilingual fluency (English/Spanish) strongly preferred, dependent on Erie site. Other bilingual fluency a plus.
