Job Title: Social Worker – Hospital Based
Location: Lawrenceville, GA 30046
Job Type: Contract
Duration: 12 Weeks
Start Date: 10/12/2026
End Date: 01/02/2027
Shift: Day | 8:00 AM–4:30 PM
Schedule: Monday–Friday | 5×8 hours | 40 hours/week
Local Candidates: Not accepted within 50 miles
Traveler Candidates: Accepted
Pay Rate: $45–$50/hr

Requirements:

2+ years recent Social Worker experience
Active Georgia LCSW license
AHA BLS
Acute hospital experience required
Hospital-based social work experience
Strong assessment and care-coordination skills

Responsibilities:

Conduct psychosocial assessments
Provide emotional support and counseling
Assist with discharge planning
Coordinate referrals and community services
Identify discharge barriers
Collaborate with physicians, nurses and case managers
Maintain accurate documentation
Support continuity of care

Holiday Requirement: 2 major holidays + 1 minor holiday.
