About the Organization Angels of Care Pediatric Home Health is a clinician‑owned and operated home health agency serving the special needs community. Positions are available on a part‑time or full‑time basis. Job Description An Occupational Therapist (OT) will develop and implement treatment programs to assist pediatric patients with physical, cognitive, sensory, and social/emotional disabilities or delays. The role involves planning and administering occupational therapy in the home and community. Responsibilities Provide high quality care and meet the needs of patients and families by performing evaluations and interpreting assessment results; develop goals appropriate for each child and create and implement occupational therapy treatment plans in conjunction with the physician. Assist pediatric and young adult patients in physical, neurological, cognitive, sensory, feeding, ADL, and social/emotional development, improving independence through skilled interventions. Educate, coach, and empower caregivers and families to carry over therapeutic activities into the child’s daily routine. Coordinate with referral partners to provide services for children in accordance with physician orders. Ensure continuity of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs. Document patient care services and care coordination in an intuitive electronic medical record system. Maintain patient confidentiality and uphold patient confidence. Requirements State license.
