Responsibilities

• Perform patient evaluations and develop individualized treatment plans based on physician orders
• Deliver physical therapy treatments and adjust plans as needed to achieve functional goals
• Supervise PTAs, rehab techs, and therapy students as required
• Maintain accurate and timely documentation in accordance with regulatory standards
• Collaborate with interdisciplinary team members, including nurses, physicians, and occupational therapists
• Educate residents, families, and staff on therapeutic techniques and home exercise programs
• Participate in discharge planning and facility quality improvement initiatives

Qualifications

• Valid state licensure as a Physical Therapist
• Prior experience in long-term care is a plus
• Strong communication and organizational skills
• Passion for working with the geriatric population
