Description: The RN implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Coordinating, ensuring, and providing direct patient care activities on the unit involves the provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Providing patient and family-centered education and care coordination with individualized treatment planning as applicable is required. The RN performs and documents a comprehensive assessment in accordance with scope of practice and hospital policies and procedures. Consideration of the patient's culture, age, and developmental level is crucial. Identifying patient risk factors and presenting complaints is necessary. Obtaining patient health information from family or others as necessary is part of the role. Collecting objective and subjective data is required. Verifying initial admission assessments obtained by other members of the healthcare team is part of the responsibilities. Developing a problem list and prioritizing patient needs is essential based on input from the inter-professional team as appropriate. Designing actions and interventions relevant to the patient’s culture and age is necessary. Determining desired outcomes in coordination with the patient and family in accordance with the medical plan of care is required. Fully implementing patient care therapies and procedures within established policies, procedures, and professional guidelines is essential. Explaining procedures, treatments, and processes to the patient and family is crucial. Initiating action to alleviate the patient’s stress and discomfort is part of the role. Recognizing, responding to, and communicating changes in patient condition appropriately is necessary. Conducting and documenting ongoing assessments and responses to procedures and therapies is required. Delegating tasks appropriately and overseeing team members in the performance of activities regarding patient care is essential. Monitoring the accuracy and completeness of delegated patient care is part of the responsibilities. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is important. Facilitating the patient discharge process by executing discharge orders in a timely manner is required. Providing input into patient/nurse assignments in consideration of available staffing, unit activity, and patient acuity is necessary. Demonstrating active participation in unit-based activities in support of organizational values and priorities is expected. Promoting a positive group dynamic and healing environment through words and actions is essential. Assuring nursing excellence in accordance with accepted standards, evidence-based practice, and current research is required. Actively participating in department or unit quality improvement activities and core measures is necessary. Using a holistic perspective regarding patient care is essential.
