Job Title Home Health RN Hours / Shift Days, 8:30‑5p. Rotating holidays (1 summer/1 winter that rotate). Every 4th weekend, including Saturday and Sunday. Summary Penn Medicine at Home offers two tracks for RN employment. This position may be filled at the Clinical Nurse – NCL or Clinical Nurse II track, depending on degree level and years of experience. Responsibilities The Clinical Nurse 2 or Clinical Nurse‑NCL is an RN who practices as a full partner on the health‑care team and consistently demonstrates safe practice within the organizational professional practice model. Possesses the knowledge, skills, and the attitude to effectively meet standards for competent professional nursing practice as specified in the position description addendum. Exhibits UPHS organizational behavioral competencies that reflect its mission and are embedded in the professional practice competency domains. Independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients, who may range in age from infancy to the elderly. Demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family‑Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork. Works closely with the interprofessional health‑care team to facilitate coordination of care across the continuum, focusing on individualized care based on physical, psychosocial, cultural, educational, safety, and age‑appropriate considerations of assigned patients. Seeks feedback in assuming leadership roles and provides feedback for improved clinical practice, demonstrating how nursing practice impacts the organizational vision, mission, and goals, and complies with all regulatory and accrediting standards. Professional Practice Competency Domains Continuous Quality Improvement (CQI): Utilizes data and QI methods to identify potential and actual problems and opportunities to provide safe, timely, efficient, effective, and equitable care. Minimum Requirements for Clinical Nurse‑NCL Track Diploma or Associate Degree in Nursing (ASN) from an accredited school of nursing. 15+ months of relevant professional nursing experience. Pennsylvania (or New Jersey) RN licensure. BLS/CPR as a healthcare provider, as per the American Heart Association. Valid driver’s license, car, and car insurance. Minimum Requirements for Clinical Nurse II Track Bachelor of Science in Nursing (BSN) from an accredited school of nursing. 15+ months of relevant professional nursing experience. Pennsylvania (or New Jersey) RN licensure. BLS/CPR as a healthcare provider, as per the American Heart Association. Valid driver’s license, car, and car insurance.
