Home Health Physical Therapist Assistant (PTA)

One Home Health is actively seeking compassionate, dedicated, and motivated Physical Therapist Assistants (PTAs) to join our growing home health team throughout Illinois.

We currently have urgent needs in the South Suburbs, Downtown Chicago, North Side Chicago, and Lake County , with additional territories available across Illinois. We strive to provide manageable coverage areas whenever possible based on clinician location and patient needs.

Compensation

Job Type: Full-Time, Part-Time, PRN/Per Diem
Pay: $45.00 - $55.00 per hour
Location: Illinois (Urgent Needs in South Suburbs, Downtown Chicago, North Side Chicago, and Lake County)

Compensation is based on experience, qualifications, and assigned territory.

Key Responsibilities

As a Home Health Physical Therapist Assistant, you will:

• Deliver physical therapy treatments and education to patients in their homes under the direction of a licensed Physical Therapist and according to the physician-approved plan of care.
• Assist patients in improving mobility, strength, balance, endurance, and functional independence.
• Observe, document, and report patient responses, progress, concerns, and changes in condition to the supervising Physical Therapist.
• Complete all required clinical documentation accurately and within agency timelines and compliance standards.
• Educate patients and caregivers on therapeutic exercises, fall prevention, home safety, and treatment goals.
• Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate patient care.
• Participate in case conferences, interdisciplinary meetings, and care planning discussions as needed.
• Follow infection prevention protocols, safety procedures, and equipment guidelines during patient visits.
• Maintain patient confidentiality and adhere to all agency, state, federal, and professional practice requirements.
• Attend staff development programs, training sessions, and quality improvement initiatives.

Qualifications

Required

• Active Illinois Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association (APTA).
• Minimum of six (6) months of PTA experience.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, documentation, and interpersonal skills.
• Ability to travel to patient homes within assigned service areas.
• Ability to work independently while maintaining high standards of patient care.

Preferred

• Home health experience preferred.
• Experience working with geriatric and rehabilitation patients.
• Knowledge of Medicare home health documentation requirements.
• Experience with Axxess software.
• Bilingual candidates are encouraged to apply.
