Oncology Nurse Navigator – Rapid Clinic (950056) Job Summary: The Oncology Nurse Navigator supports the new RAPID clinic (Rapid Assessment for Potential Incidental or Diagnosed Malignancy). The Navigator contacts patients within 24 hours of referral, develops relationships, assesses social determinants of health, and ensures all records and documentation are available for the patient’s first clinic visit. The role navigates patients toward diagnosis, coordinates hand-offs to the care team, and provides ongoing education and resources to facilitate informed decision‑making. Responsibilities Assesses barriers to care across the oncology continuum. Serves as point of contact for providers, patients, and caregivers. Advocates for patients and caregivers through the healthcare system. Facilitates education of patients and staff by developing appropriate programs, resources, and tools. Provides education, resources, and referrals to internal and external support services based on assessment needs. Facilitates shared decision‑making, ensuring patients, families, and caregivers are fully informed of treatment intent, options, and outcomes. Promotes care planning by encouraging patients to articulate wishes and goals of care; works with patients to formulate questions for providers. Facilitates clear and concise communication among providers, departments, and patients. Coordinates multidisciplinary planning and facilitates care conferences as needed. Participates in outreach projects to close patient care gaps. Performs other duties as assigned. Qualifications Education : Associate’s Degree graduate of an approved/accredited nursing program approved by a governmental authority, nursing board, or licensing authority (Texas BON 217.1). Experience : 3 years oncology nursing experience. Licenses and Certifications : Registered Nurse (RN) and Basic Life Support (BLS) course accredited by the American Heart Association (AHA) or American Red Cross (ARC). Preferred : Education: Bachelor of Science in Nursing. Licenses and Certifications: Oncology Certified Nurse (OCN), Certified Physician Assistant in Oncology (CPHON), Certified Breast Care Nurse (CBCN), or Blood‑Marrow Transplant Certification RN (BMTCN).
