Registered Nurse (RN) - Hospice
Community Hospice
Registered Nurse – Hospice and Palliative Care
7 on/7 off, with flexibility to cover additional territories as needed. Shifts are Saturday and Sunday from 8 AM to 8 PM or 8 PM to 8 AM, and Monday to Friday from 5 PM to 8 AM.
Assignment Details
Standard 40 hours per week with on-call responsibilities.
Requirements
- Ample experience in hospice care
- Experience with on-call and admissions
- Proficiency in using the HCHB electronic medical record system
Responsibilities
- Assess and address physical, psychosocial, and environmental needs of patients and families
- Document nursing assessments, care plans, interventions, and outcomes
- Develop individualized pain management plans and manage symptoms
- Participate in interdisciplinary team meetings to revise care plans
- Educate patients and families on care interventions and medication management
- Provide holistic, family-centered care and support unique spiritual and cultural beliefs
