You’ll be working in close quarters with nurses, aides, and specialists-oriented teams, ensuring streamlined operations and care delivery.

Key Responsibilities

• Conduct detailed and comprehensive assessments to determine the health status of residents, including physical examinations and health histories.
• Develop, implement, and evaluate individualized care plans based on evidence-based practices in collaboration with interdisciplinary teams including physicians, physical therapists, and social workers.
• Order, interpret, and manage diagnostic tests—such as blood work and imaging—as well as initiate timely interventions for acute and chronic conditions.
• Prescribe medications and therapies while monitoring resident responses, adjusting protocols as necessary to optimize recovery and well-being.
• Educate residents and their families about health management, medication adherence, and strategies for improving lifestyle choices to enhance overall health results.
• Participate actively in multidisciplinary meetings, ensuring comprehensive care discussions and joint decision-making on resident care paths.
• Maintain up-to-date and accurate electronic medical records, documenting assessments, treatment plans, and care outcomes in accordance with facility policies.
• Engage in quality improvement initiatives and contribute to the development of best practice guidelines as part of our commitment to excellence in care.

Patient Population / Client Focus

Your primary focus will be on a geriatric population with a range of health conditions, including Alzheimer's, diabetes, hypertension, and other age-related ailments. Understanding the complexities of these diseases, alongside the social and emotional needs of our residents, is essential. You will be a vital advocate in their healthcare processes, ensuring their voices are echoed through their treatment plans.

Qualifications

• A current Nurse Practitioner certification and an active state nursing license in Florida.
• A Bachelor’s degree in nursing; a Master’s degree in Nursing or a related field from an accredited institution is preferred.
• A minimum of 2 years of clinical experience in a similar setting, such as assisted living or geriatric care.
• Strong communication skills to foster relationships with residents, families, and medical staff.
• Proficiency in using clinical systems and electronic medical records (EMR) for documentation and communication.

Preferred Experience

• Experience in an assisted living or nursing facility environment will be highly valued.

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary range of $120,000 to $220,000 annually, based on your experience and qualifications.
