Looking for a full-time Physical Therapist for a Skilled Nursing Facility in Rochester, NY.

Work Location: Waterview Heights Rehabilitation & Nursing Center

135 Meriden St., Rochester, NY 14612

Job Responsibilities:

• Evaluates, provides, and ensures patient’s plan of care.

• Oversees Physical Therapy Assistants providing guidance, discussing progress and monitoring clinical interventions/skills.

• Provides skilled services and interventions using various techniques that are needed to achieve patient’s goals.

• Follows all regulations and guidelines of facility including compliance with confidentiality and HIPPA.

• Adheres to discussed scheduled hours and conducts self in a professional manner at all times.

• Provides skilled documentation of patient’s condition, and provide caregiver/patient education, progress and need for services.

• Ensures safe treatment practices and monitors patient’s engagement and responses to treatment, modifies as necessary.

Requirements include:

• Valid NY professional license

• Graduated from an accredited Physical Therapy program

• Preferred experience with rehab optima and skilled nursing facilities

• New graduates are welcome
