Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting tailored treatment plans based on individual needs
• Implement physical therapy interventions that target mobility, strength, balance, and overall functionality
• Work closely with a diverse team, including nurses, physicians, and case managers, to ensure optimal patient outcomes
• Guide and educate patients and their families on therapeutic objectives, discharge plans, and at-home exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in accordance with facility and regulatory standards
• Engage actively in multidisciplinary team rounds and conferences to promote coordinated and holistic care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialized areas (neurological, geriatric, etc.) are advantageous but not mandatory

• Experience in physical therapy is highly favored
