Clinical Interviewing- Yes – manager to conduct final interview

On Call, Call Back Requirements: Call is not required, but may be available if interested

Weekend Rotation: None, unless traveler is willing and interested in working weekends ▪ Unless posted for Weekend only Work

Holiday Requirements: Travelers will be expected to work all holidays, unless approval is received from Manager

About Facility:

• Home Health
• Radius rule: Traveler's permanent address must be 50 miles outside of home unit

Department Information:

• Will make visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and cities in between
  • There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
  • Best central location for housing University City and/or Concord but must be comfortable to float throughout the areas above.
• Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)

Minimum Years of Experience required: 1 year Home Health Nursing

• Will you accept a first-time traveler: Yes, if seasoned to home health

Patient Types/Common Diagnoses:

• Chronic diseases, wound care/wound vac, IV, pleurx

Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:

• Driver's License and proof of Insurance
• BLS

Skills required:

• Charting experience with Homecare Homebase is required (no exceptions), IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)

Support within the Department:

• Departmental leadership
• Schedular
• Clinical Manager
• Clinical Supervisor
• Peers/Clinical Team

Technology/Equipment:

• EPIC Rover – view only
• Focura - will upload discharge instructions and other important docs such as wound pictures
• Samsung note 9 phones
• Home Care Home Base – experience required

Orientation (facility and unit):

• NEO
• 5 days orientation with Preceptor

Scheduling:

• Weekend rotation: none, unless traveler is willing and interested in working weekends

• Call is not required, but may be available if interested
• Holiday Expectations: Travelers will be expected to work all holidays, unless approval is received from Manager
• RTO: Manager approval
• Schedule cycle: every 13 weeks

• Do they schedule their own appointments with this position: - they will schedule the visit with the patients the day prior to the visit planned.
• Do they still get paid for cancelled appointments: cancelled appts is not counted as visit equivalents. They are required 30 VE a week

• Are there any clinic days: (I have no idea what they are referring to) each branch is a little different. 1x week in person and at times virtual
• Is a company cell phone provided: A phone is provided and is used for documentation and calling patients/providers.
