Physical Therapist PRN

Role: Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing evidence-based treatments to enhance mobility, independence, and quality of life. Responsibilities Deliver one-on-one therapy sessions in private residences and senior communities Perform evaluations, develop care plans, and implement treatments Document in EMR systems and collaborate with interdisciplinary teams Educate patients, caregivers, and staff to promote safety and aging in place Utilize telehealth and virtual care to extend service accessibility Qualifications & Skills Valid Physical Therapy license or eligibility in the state of practice Degree from an accredited Physical Therapy program Proficiency in computer use and EMR systems Current CPR certification Physical Requirements Ability to perform moderate physical effort, assist with patient transfers, repositioning, and therapeutic activities; capable of driving and wearing PPE as needed.
