As a Home Health Speech Language Pathologist at CenterWell Home Health, reporting to the Clinical Manager, you’ll play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you’ll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life. Responsibilities Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions. Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening. Observe, record, and report changes in the patient’s condition and response to treatment to supervisor and/or physician. Provide instruction and training to patients in the use of alternative communication systems when appropriate. Provide counsel and instruction to patients, families, and healthcare staff. Maintain and submit documentation as required by the Company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy. Participate in care coordination activities and discharge planning. Maintain the highest standards of professional conduct concerning confidential information. Qualifications Minimum six months experience as a speech therapist / speech language pathologist. A valid driver’s license, auto insurance, and reliable transportation are required. Home health experience. Pay & Benefits Pay: $63.00 - $88.00 per visit/unit.

• This role is part of Humana’s driver safety program and requires an individual to have a valid state driver’s license and maintain personal vehicle liability insurance.
• The compensation range reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
