Hospital based inpatient Clinical Care RN with $15K Sign on Bonus Location: New York,NY, US Date Posted: 08-30-2026 Job Description Experience: Mid Level Salary: $101,500 - $120,000 per year Job Details This is a full time ONSITE position at a MANHATTAN Hospital. The work schedule is Monday to Friday from 9 am to 5 pm with 1 weekend per month. RN will always work 5 days every week, 8-hour shifts (can be 8a to 4 pm or 9a to 5 p or 10am to 6 p). RNs should be prepared to work within the Hospice referral sources both within the hospital setting and in the outpatient setting (MD offices, clinics, SNFs, etc.). Serve as a key intermediary between Hospice organization, hospital staff, patients, and their families to ensure clear and effective communication and coordination of hospice care, Business development. Conduct thorough chart assessments of patient cases and bedside evaluations to determine the clinical picture and hospice eligibility, Collaborate with facility clinical team and hospice physicians to make informed decisions regarding patient care, Successfully facilitate numerous hospice admissions, ensuring seamless transitions for patients and their families, Provide critical support and education to enhance patient and family understanding of hospice care, leading to improved satisfaction and care outcomes, Oversee the coordination of admission assessments and the procurement of durable medical equipment, Disposition Planning, Ensuring that all patient needs are met.
