Key Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs that support effective return to community activities, progressing patients toward discharge within an established length of stay
• Delegate and direct discipline-specific therapy services for assigned patients; serve as an educator for patients, families, students, and coworkers
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations

Required Qualifications

• Active Physical Therapist license in good standing in the state of Colorado
• Experience performing 60-minute evaluations and 30-minute follow-up visits in an outpatient setting
• Ability to float across multiple clinic locations
• Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
• Commitment to patient and employee safety standards and adherence to clinic policies and procedures

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students
• Familiarity with performance improvement methodologies and quality initiatives
