Outpatient Physical Therapist - Longmont Clinics

This position offers a 5x8.0 shift, including time split between both Longmont clinics.

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to patient needs and physician directives. You will be responsible for the delegation and direction of all therapy services for assigned patients, ensuring functional treatment programs that enable effective community reintegration. The role includes educating patients, families, and colleagues while performing professional duties related to patient care and service development.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans aimed at timely patient discharge.
• Document patient care clearly, accurately, and promptly in accordance with clinic policies.
• Provide care under valid physician orders and within insurance guidelines.
• Participate in student education and facilitate a positive learning environment.
• Create and pursue an individualized professional development plan.
• Adhere to state practice regulations and maintain professional licensure.
• Engage in performance improvement initiatives and contribute feedback for maximized outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months to 1 year of outpatient clinical experience.
• Current CPR certification required.
• Maintained licensure in good standing.
