Team Specialist

As a Valleywise HealthBehavioral Health Team Specialist , you'll work directly with adults living with serious mental illness (SMI) , helping them build stability, independence, and meaningful connections within their communities. You'll provide hands‑on case management, rehabilitation, support, and crisis intervention services both in the clinic and throughout the community. This is an opportunity to build meaningful relationships, remove barriers to care, and help members take important steps toward their individual recovery goals. What You'll Do Provide case management, rehabilitation, support, and crisis intervention services to members and families. Meet members where they are, including in the clinic, their homes, places of employment, and throughout the community. Build supportive relationships with adults living with serious mental illness. Collaborate with members and the care team to develop and implement Individual Service Plans (ISPs) based on each person's needs and recovery goals. Coordinate behavioral health services and community resources identified in each member's service plan. Help members navigate available programs, benefits, entitlements, and community resources. Provide services using evidence‑based, trauma‑informed, and recovery‑oriented approaches . Collaborate with members, families, clinicians, and other members of the multidisciplinary care team. Respond appropriately to behavioral health needs and crisis situations. Maintain accurate, confidential information while following HIPAA requirements. Work independently in the community while seeking clinical guidance and supervision when appropriate. Hourly Pay Range: $19.73 - $29.10 Qualifications Education Bachelor's degree in a field related to Behavioral Health or Any other Bachelor's degree and one (1) year of full-time behavioral health work experience or Any Associate's degree and two (2) years of full-time behavioral health work experience or Eighteen (18) credit hours of post‑high school education in a field related to behavioral health and two (2) years of full-time behavioral health work experience or High school diploma or GED and four (4) years of full-time behavioral health work experience. Experience Requires a minimum of one (1) year of experience working with adults with a serious mental illness, demonstrating an understanding of the required knowledge, skills, and abilities. Previous experience working with individuals with psychotic disorders is preferred. Certification/Licensure This position requires operation of a vehicle. Employment is contingent upon meeting the driving eligibility standards outlined in the Valleywise Health Vehicle Use Policy (VUP). Requires possession of and ability to retain a current valid Arizona‑issued driver's license in good standing and must carry the AZ legally required minimum insurance coverage on personal vehicle at all times. Must obtain fingerprint card as defined in Arizona Revised Statute (A.R.S.) 36-425.03 and state licensing rule R9-10-1011(A)(4) within 7 days of employment. Pursuant to Arizona Administrative Code R9-10-306 CHAPTER 10 of the Dept. of Health Services, personnel must be at least 21 years of age, or at least 18 years of ageand licensed or certified under A.R.S.
