What You’ll DoDeliver hands-on nursing care in the patient’s home
Monitor vitals, manage medications, and follow the care plan
Provide comfort, safety, and support to the patient and family
Document care accurately and promptly
Active LVN license
Current CPR certification
- Tracheostomy and Ventilator certification
Compassion, professionalism, reliability
Home health or shift care experience a plus
Flexible shifts (days, nights, weekends)
Supportive clinical leadership
