Job Description:

InGenesis is currently seeking Licensed Practical Nurses (LPNs) to work with our client. In this role, you will provide direct and indirect nursing care to approximately 50 to 60 geriatric and/or disabled veteran members under the direction of a Registered Nurse (RN). LPNs play a critical role in delivering high-quality, resident-centered care that promotes the health, dignity, comfort, and well-being of our members.

Essential Job Duties

• Provide direct and indirect nursing care to 50 to 60 geriatric and/or disabled members under the direction and supervision of an RN.
• Measure, administer, and accurately document medications in accordance with physician orders, nursing standards, and facility policies.
• Monitor residents' conditions and promptly report significant changes to the RN or other appropriate healthcare professionals.
• Perform LPN-level nursing procedures, including:
  • Tube feedings
  • Catheterizations
  • Irrigations
  • Sterile dressing changes
  • Suctioning
  • Tracheostomy care
  • Nebulizer treatments
  • Specialized skin care and wound care
• Assist with the development, implementation, and evaluation of individualized care plans.
• Maintain accurate and timely nursing documentation.
• Provide resident and family education regarding treatments, medications, and care plans.
• Collaborate with physicians, RNs, CNAs, therapists, and other interdisciplinary team members to ensure quality resident care.
• Adhere to infection control practices, safety protocols, and all state and federal regulations.
• Perform other duties as assigned.

Minimum Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in the State of Wisconsin.
• Ability to administer medications and perform nursing procedures within the scope of LPN practice.
• Knowledge of nursing principles, practices, and standards applicable to long-term care settings.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary healthcare team.
• Ability to maintain accurate records and documentation.

Preferred Qualifications

• Experience working in a skilled nursing facility, long-term care setting, rehabilitation center, or veterans' home.
• Experience caring for geriatric and disabled populations.
• Knowledge of chronic disease management, wound care, and rehabilitative nursing practices.
