The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our rehabilitation team. The Occupational Therapist will evaluate residents and develop individualized treatment plans focused on improving daily living skills, functional independence, and overall quality of life.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide therapeutic interventions to improve strength, coordination, mobility, and activities of daily living
• Assess resident progress and modify treatment plans as appropriate
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards
• Educate residents and families regarding therapy programs, adaptive equipment, and treatment goals
• Participate in care planning meetings and discharge planning processes
• Ensure compliance with all federal, state, and facility policies and procedures
• Maintain a safe, organized, and supportive therapy environment

Qualifications

• Active Florida Occupational Therapist (OT) license in good standing
• Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Ability to work effectively within an interdisciplinary team
• Compassionate, dependable, and resident-focused
