The Hospice Chaplain fulfills an essential role in developing a holistic plan of care to meet the individualized goals of the patient and family. The Chaplain’s expertise lies in identifying and addressing issues related to spiritual, religious, and emotional needs in advance illness at end of life. Quality Provision of Services: Aligns all professional practice with Capital Caring mission to provide the highest quality advanced illness care with Dignity, Respect, and Compassion by incorporating Capital Caring Health values to provide person-centered care, treat everyone with respect, and to practice with integrity. Functions as a member of the inter-disciplinary team (IDT), reporting in a timely manner any observed/reported changes in patient/family condition to appropriate members of the team. To include, collaborating on care planning and coordinating provision of care. Performs religious services of prayer, worship and rituals for patients, families and staff as requested. This may include conducting patient memorial services and funerals, Baptisms, offering Communion, and other services as indicated. The Chaplain provides spiritual support and acts as a resource to members of the IDT, as needed. The Chaplain seeks opportunities and is expected to provide education to the community-at-large. The Chaplain participates in all scheduled staff, organizational, or discipline-specific meetings. Demonstrates flexibility with work schedule, including working evenings/weekends as needed to meet the needs of patients, staff, and the organization. Chaplain participates in hospice sponsored in-service training and continuing education opportunities. Chaplain may also participate in community sponsored training opportunities as directed. Serves on both internal Capital Caring Health and external Community committees or teams as approved by organizational leadership. Participates in orientation of all new-hires, interns and volunteer staff as requested. Chaplain regularly provides Family Services On-Call coverage as directed for evenings, weekends, and holidays. Chaplain collaborates with facility staff upon each patient visit and documents visit details in accordance with facility protocol. Organization/Regulatory Compliance: Performs spiritual assessment within 5 days of the patient’s admission to determine the spiritual needs and strengths of the patient/family. Develops a plan of care with patient/family, the physician, the nurse, the social worker, and other members of the interdisciplinary team (IDT) that is individualized to meet the patient/family spiritual needs. a. Identifies problems, goals and interventions and revises and updates them based on input and a reassessment every 14 days (two weeks). b. The spiritual/pastoral plan of care: i. Demonstrates an effort to work in close collaboration with local clergy ii. Provides spiritual support as needed and defined by the patient/family Uses approved forms and assessment/documentation tools according to Capital Caring Health policies and procedures. Completes documentation for each visit on day of visit with preference to document at time of visit. Follows all rules and regulations as required when visiting patients in facilities. Complies with facility documentation requirements. People/Communications: Participates in and contributes to the IDT through regular attendance, active discussion of patient and family needs, and routine collaboration and/or joint patient visits conducted with members of the IDT. Acts as a communication bridge between patient/family and the IDT to facilitate understanding of the psycho-spiritual needs and any issues related to patient care. Offers patients/families of differing philosophies and religious beliefs the opportunity to discuss and explore their thoughts, feelings, and values. Develops and maintains effective relationship with community clergy and spiritual leaders. Fosters and maintains partnerships with faith-based community groups with the aim of assessing faith community needs and developing action plans geared towards raising awareness of Capital Caring Health services. Regularly participates in speaking engagements in collaboration with organizational leadership. Routinely collaborates and communicates with facility staff, performs requested spiritual services and provides education as requested. Financial Stewardship: Achieves expected productivity standard (to include patient visits and caseload) as defined by the organization and in alignment with best practices. Supervisory Responsibilities: The Chaplain will only perform Supervisory duties when recommended and educated to do so by organizational leadership. Experience Requirements The Chaplain must have at least three years of pastoral or pastoral counseling work. Experience in working with families and caregivers facing death and bereavement is preferred. The Chaplain must have a Master of Divinity degree from an accredited professional theological school or seminary involving at least three years of post-bachelor education, or an equivalent graduate degree with professional pastoral counseling training. Ordination and/or official endorsement by the candidate’s faith community or the candidate’s ministry is required. Required Certificates and/or Licenses: The Chaplain will have completed four certified units of clinical pastoral education in an accredited hospital (or the equivalent of such education); professional certification as a chaplain by a nationally recognized certifying organization; and/or an ecclesiastical endorsement for chaplaincy by the faith community. Preference for Licensed Pastoral Counselor or equivalent license. Overall Skills: The ability to demonstrate superlative oral and written communication skills, in English, as well as ability to organize and manage projects within defined timeframe and budget; work both independently and as part of a team; demonstrate excellent interpersonal and collaborative skills; demonstrate organizational skills with ability to delegate and follow up; manage multiple tasks and stressful situations with a positive attitude; demonstrate a strong work ethic and attention to detail; high degree of accuracy in typed work; excellent proofreading skills; ability to communicate effectively with various levels of management, peers, vendors, and public; demonstrates functional knowledge of religious principles and practices of the major world faiths. Physical/Mental/Emotional Skills: The ability to see, hear, grasp, lift, speak, sit and walk or otherwise perform essential functions with or without reasonable accommodations; demonstrate the mental and emotional ability to work with death/dying; demonstrate the maturity, customer service skills, and flexibility to work with individuals under stress; work in a fast paced and quickly changing environment; work under pressure and deadlines; effectively recognize and appreciate the diversity in the workplace; demonstrate personal maturity, emotional stability, and physical energy appropriate to Hospice needs in order to handle the stress of working with dying patients, grieving survivors, and other team members who are also under stress. Technological Skills: The ability to learn and effectively utilize applicable patient health information systems. Reasoning Ability: The ability to apply common sense understanding to carry out instructions furnished in written or oral form to manage complex patient and family situations. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is occasionally required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; taste or smell and push, pull, lift/carry 35 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The Chaplain is required to drive between patient settings to include driving in the evenings/night and in snow/wet conditions with distances of an hour or more. Work Environment: The work environment characteristics are representative of those an employee would normally encounter while performing the essential functions of this job in an office. The noise level in the work environment is usually light to moderate. The environmental conditions are that of what is to be expected in a temperature-controlled building. Compensation Pay Range: $55,580.29 - $82,725.70 This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
