Job Summary The RN Case Manager leads the complex discharge planning process and collaborates with physicians, other healthcare team members, and MSW to assess, plan, implement, and evaluate proactive and safe discharge plans. The role includes coordinating care, ensuring compliance with policies and procedures, and serving as a resource for medically complex cases. Qualifications Associate’s Degree in Nursing required; Bachelor’s Degree, BSN preferred. Two or more years of healthcare experience preferred; case management or related experience preferred. Current RN licensure in the state of employment required. Weekend and holiday coverage expected. Knowledge of Microsoft Office products and keyboarding preferred. Competencies Patient Care Interview and advise patients on health and illness prevention. Synthesize data to determine preliminary diagnosis and therapeutic plan. Recommend community resources to meet patient and family needs. Quality Collaborate with other departments to achieve quality outcomes. Monitor core processes and apply safety and infection control principles. Communication Demonstrate superior interpersonal and communication skills. Collaborate with team to achieve patient and team goals. Initiate regular, professional communication with coworkers. Participate in staff meetings and identify process improvement opportunities. Medical Knowledge Provide primary health care and selective medical services under physician direction. Obtain patient history, review records, interpret data, and determine diagnostic and therapeutic procedures. Perform complete physical exams, order labs, and provide continuity of care between visits. Physical Demands Medium‑to‑heavy demand: Requires constant standing, frequent walking, bending, stooping, and the ability to lift/push/pull up to 100 lbs. Safety handling equipment should be used for any weight over 35 lbs. The role also demands proficient communication, auditory, and visual skills, attention to detail, and legible writing. Additional Requirements Use of safety equipment (e.g., HEPA mask) may be required. On‑call responsibilities as directed. Ability to travel between campus buildings, remote facilities, and out of town as needed. Compensation The base compensation range for this role is $32.12 – $50.50 per hour. Individual compensation is determined by the selected candidate’s qualifications, previous work experience, and/or education.
