Contract | 13 Weeks | Days | Onsite | Monday 6:30 AM–12:30 PM, Tuesday/Thursday 9:00 AM–7:00 PM, Wednesday 6:30 AM–3:00 PM, Friday 7:00 AM–3:30 PM

Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs that support effective patient return to community activities within an established length of stay
• Delegate and direct discipline-specific therapy services for assigned patients
• Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Virginia
• Minimum of 6 months of clinical experience as a Physical Therapist
• Proficiency in outpatient physical therapy evaluation and treatment
• Ability to document accurately and in compliance with state, federal, and payer-specific guidelines
• Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards

Preferred Experience / Skills

• Experience in an outpatient rehabilitation setting
• Familiarity with performance improvement methodologies and quality initiatives
• Active involvement in continuing education and professional development activities
