Reema Health is hiring a Community Health Navigator to build relationships with members, guide them toward engagement in their healthcare, and address social barriers. You will work remotely/field-based, reach out via phone and in-person visits, and document activities in the Reema App. Responsibilities include enrolling members, managing a caseload, coordinating referrals, and hitting monthly goals while maintaining confidentiality and HIPAA standards. #J-18808-Ljbffr Reema Health
