ABOUT THE JOB
We have an opening for a PER DIEM (field-based) California-Home Health Registered Nurse (RN) to provide skilled nursing visits to patients in Long Beach and surrounding areas . This role requires strong clinical judgment, efficient documentation, and the ability to work independently in the field.  

RESPONSIBILITIES
* Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits.

* Conduct comprehensive patient assessments and develop individualized care plans.

* Complete accurate and timely documentation (within 48 hours compliance)

* Perform medication reconciliation at each visit and report changes.

* Provide patient and family education based on diagnosis and care needs.

* Collaborates with Care Coordinators, and interdisciplinary team.

* Maintain compliance with Joint Commission, CDPH, and agency standards.

QUALIFICATIONS
* At least one year of nursing experience, including in a Home Health or similar environment is generally required (knowledge in OASIS documentation is a plus)

* Strong clinical, organizational, and communication skills.

* Ability to work independently and manage time efficiently.

* Familiarity with EMR systems (MyUnity experience is a plus)

CREDENTIALS REQUIRED

* Active California RN License

* Current CPR

* Driver’s License

* Car Insurance

* Social Security Number

* Current Physical Exam and TB Test 

* Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
