Support discharge planning by identifying and addressing psychosocial barriers. Deliver crisis intervention, grief counseling, abuse reporting, and other urgent psychosocial interventions. Maintain timely, accurate documentation in accordance with department and regulatory standards. Connect patients and families to community, legal, and clinical referral resources. Facilitate family meetings and participate in interdisciplinary rounds and quality improvement efforts. Participate in required supervision per COMAR regulations. Masters degree in applied psychology or a related field Licensure/Certification: Maryland LGPC or LGSW-C required Experience: 1 year of healthcare-related experience preferred Field placement may be considered Salary Range: Minimum 32.40/hour - Maximum 53.48/hour.
