Tackle non‑medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)—such as food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment. Provide Emotional Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services. Provide advocacy & resource expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well‑being post‑discharge. What Minimum Requirements You’ll Need Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred; ACM also accepted. Certified Social Worker preferred. Education Bachelor's degree in Social Work required. Work Experience 3 years of experience preferred.
