Physical Therapist (PT) – Home Health

Facility Type: Home Health

Must Have

• Active Physical Therapist License
• BLS Certification
• Home Health Physical Therapy experience
• Experience performing patient evaluations and functional assessments
• Experience developing individualized treatment plans
• Knowledge of home safety assessments
• Strong clinical judgment and problem-solving skills
• Excellent patient and caregiver education skills
• Ability to work independently and manage a daily caseload
• Valid driver's license and reliable transportation (typically required for Home Health)

Preferred

• Previous Home Health travel experience
• Experience treating adult and geriatric patients
• OASIS documentation experience
• Experience with electronic medical records (EMR)

Schedule

• Day Shift: 8 Hours
• Monday–Friday
• Flexible Schedule: Sunday–Thursday or Tuesday–Saturday
• 40 Guaranteed Hours per Week
• Contract Length: 13 Weeks

Key Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, gait training, balance training, and mobility training
• Assess home safety and recommend adaptive equipment
• Educate patients and caregivers on treatment plans and home exercise programs
• Monitor patient progress and modify treatment plans as needed
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers
• Complete timely and accurate clinical documentation
• Promote functional independence and prevent hospital readmissions
• Ensure compliance with Home Health regulations and agency policies
