Nurse Practitioner / Physician Assistant

Position Overview Physical Medicine and Rehabilitation (PM&R) or Physiatry clinician. Work exclusively at a designated skilled nursing facility (SNF) or post‑acute rehab center in Colchester or Groton, CT. 32‑hour per week, W-2. Responsibilities Provide specialized PM&R services as a rehab consultant Represent CRC in the facility, leading clinical collaborations Collaborate with PCP teams on common SNF conditions (CVA, progressive neurological conditions, post‑operative orthopedic surgeries) Work with PT, OT, SLP teams to improve patient function and quality of life Follow therapy patients and track progress Assess and manage therapy barriers (joint/neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, etc.) Participate in multidisciplinary meetings, bridging therapy and internal medicine Engage in culture‑building and continuing education (monthly didactic lectures, virtual meetings, regional dinners) Qualifications Registered NP or APRN or Physician Assistant with valid Connecticut license Valid U.S. work eligibility Experience in post‑acute setting preferred Passionate about post‑acute and geriatric care Autonomous work style and strong interpersonal skills Empathy, compassion, and a positive attitude Commitment to continuous improvement and company culture Benefits Competitive salary starting at $120,000 and bonus Health, vision, and dental insurance Short‑term disability, life, critical illness insurance Generous PTO and flexible hours (9AM–4PM) $800 continuous education reimbursement Full‑time virtual scribe assistance ($1,368/month value) 30 CEUs at no cost; access to CRC Physiatry certification 401(k) plan with 2‑3% annual contribution 100% malpractice insurance coverage Laptop provided Position Information Location: Colchester, CT or Groton,CT (must commute or relocate). 4‑day work week (Monday‑Friday flexible), 8‑hour shifts.
